Listed below are some of the PADI forms you may need to complete in order to participate in scuba activities. Adventures in Diving and Correlating Specialty Diver Programs Statement of Risks and Liability

This liability form must be signed by anyone participating in the Advanced Open Water course or any PADI specialty course at Aquatic Adventures. Children will not be allowed to participate in this activity if their parents have not signed this form.

Bubblemakers Statement

This medical and liability form must be completed by children and their parents whenever a child participates in the PADI Bubblemakers program through Aquatic Adventures. Children will not be allowed to participate in this activity if their parents have not signed this form.

Discover Scuba Diving Liability Form

This form must be completed by everyone participating in the PADI Discover Scuba Diving program through Aquatic Adventures. Parents are required to sign this form if their minor child will be participating in this activity.

Discover Snorkeling and Skin Diving Liability Form

This form must be completed by everyone participating in the PADI Discover Snorkeling or Skin Diving program through Aquatic Adventures. Parents are required to sign this form if their minor child will be participating in this activity.

Certified Diver Experience Programs Liability Form

This form must be completed by certified divers participating in PADI experience programs through Aquatic Adventures. This includes the PADI Scuba Review course and Discover Local Diving. Parents are required to sign this form if their minor child will be participating in these activities.

Equipment Rental Agreement (AAI)

This form must be completed by anyone renting dive equipment from Aquatic Adventures. Parents are required to sign this form if their minor child will be participating in these activities.

Equipment Rental Agreement and Liability Release Form (PADI)

This form must be completed by anyone renting dive equipment from Aquatic Adventures. Parents are required to sign this form if their minor child will be participating in these activities.

Missed Classes and Withdrawal Policy Statement

This form must be signed by anyone participating in training at Aquatic Adventures. The statement identifies costs associated with withdrawing from class or failing to attend classes.

RSTC Medical Statement

This form must be completed by anyone who marks "YES" on the Discover Scuba Diving Medical Statement, the Open Water Medical Statement, and everyone enrolling in either the Divemaster or Assistant Instructor programs who does not have a completed RSTC Medical Statement on file that was completed within the last year. Physicians are required to read and sign this form before the student is allowed to participate in diving activities.

Seal Team Statement

This form must be completed by everyone participating in the PADI Seal Team program. Parents are required to sign this form when their minor child will be participating in this activity.

Standard Safe Diving Practices Statement of Understanding

This form must be completed by everyone participating in scuba courses or formal diving activities. This form must be completed each time a student enrolls in a PADI course.

Supervision of Certified Divers

This form must be completed by certified divers participating in scuba diving activities through Aquatic Adventures. This includes divers participating in AAI-led dives on Lake Michigan through Lender Charters.

Travel and Excursions Liability Release

This form must be completed by everyone traveling with Aquatic Adventures both nationally and internationally. Parents are required to sign this form when their minor child will be traveling with Aquatic Adventures.
